Campus Incidents of Concern

In the event of an emergency or criminal activity, call 911 or the Purdue University Police Department at 765-494-8221.

The “We Are Purdue” Statement of Values declares that, as Boilermakers, “we are a community that embodies integrity through the practice of honesty, humility, and accountability,” and one that “respects each other by embracing diversity, promoting inclusion, and encouraging freedom of thought and speech.”  Consistent with these values, Purdue’s Statement of Integrity and Code of Conduct calls on each member of the campus community to “act with honesty and adhere to the highest standards of moral and ethical values and principles through our personal and professional behavior.”

While it is not, as Purdue’s Commitment to Freedom of Expression recognizes, the proper role of the University “to attempt to shield individuals from ideas and opinions they find unwelcome, disagreeable, or even deeply offensive,” it is important that members of the community have ample avenues to share information about incidents that cause them concern.

There are several established reporting mechanisms for this purpose, including:

Individuals who believe that they have been impacted by an incident that is not of the type described above may use this form to report the concern. Staff from the Office of the Dean of Students and the Office of Institutional Equity may reach out to the incident reporter (if contact information is provided) for additional information and/or to connect the reporter to institutional resources, which may include information on how to make a report under one of the above mechanisms.

Please complete the form below with as much information as you are able to provide. The greater amount of information that is provided, increases our ability to review and provide appropriate resources. Reports that are submitted anonymously or with limited information will limit the amount of follow up that we are able to perform.
