Deadline approaching to register for 'Living with Diabetes' program, receive medication discounts
Those interested in participating in the Center for Healthy Living's upcoming Living with Diabetes program must register by Monday (Jan. 23).
The comprehensive program is designed to provide education and support (as well as medication discounts) to individuals who have been diagnosed with type 1 or type 2 diabetes or identified as someone with pre-diabetes. The program is available at no charge to benefits-eligible faculty and staff at all Purdue campuses -- including those who have opted out of Purdue medical coverage -- and for spouses and adult dependents covered on a Purdue medical plan. In addition, parents of minor children (covered on a Purdue medical plan) may participate on behalf of the child. Staff at regional campuses can use telephonic and portal support to complete the program.
The Living with Diabetes program is also open to parents and guardians of children under the age of 18 (covered on a Purdue medical plan) who have been diagnosed with type 1 or type 2 diabetes who would like to attend on behalf of their child. The same plan provisions in regard to medication discounts would apply to the child.
The program consists of a pre-assessment, a post-assessment, a 12-week class that meets once a week and ongoing health coaching and pharmacist visits tailored to participants’ needs.
Classes will run Mondays, Feb. 6 through May 1, and Thursdays, Feb. 9 through May 4.
Participants will enroll for one of the following class meeting times.
* Mondays, noon-12:50 p.m. -- Córdova Recreational Sports Center (Co-Rec), Wellness Conference Room, Room 226.
* Mondays, 2-3 p.m. -- Córdova Recreational Sports Center (Co-Rec), Wellness Conference Room, Room 226.
* Thursdays, 8-9 a.m. -- Beering Hall, Room B2016.
* Thursdays, noon-12:50 p.m. -- Pao Hall, Room B157.
Each class time is limited to 20 participants. Purdue employees who work during the program’s meeting and appointment times may participate in paid status with supervisor approval and don’t need to take sick leave or vacation.
To register, those interested can use the patient portal by following these steps:
* Log in and click the envelope icon in the top right corner of the page.
* Click the New button in the top right corner of the page.
* Click the Add Recipient button and select Wellness Program Registration, then click Select.
* Type Workshop Registration as the subject.
* Enter your name, contact information and which workshop you would like to attend in the body of the email and click Send.
Employees without computer access can call 765-494-5505 to register.
