Get a free physical, earn Healthy Boiler incentive

A starting point to good health is information – the kind that comes from a physical with a health care provider. Thanks to the 2019 Healthy Boiler Wellness Program, employees can find out valuable health information and earn an incentive at the same time.

Those who complete a physical and biometrics screening between Oct. 1, 2018, and Sept. 30, 2019, can earn $325 (employee only) or $650 (employee + spouse, employee + child or employee + family). Both the physical and biometric screening are provided at no cost to employees on all Purdue medical plans as preventive benefits.

The annual physical is a comprehensive exam that is prevention-focused, not problem-focused. Typically the exam is performed by the primary care provider and includes the following:

  • Past medical, social and family history.
  • Complete physical exam and review of body systems.
  • Review of medications.
  • Immunizations.
  • Counseling, anticipatory guidance, risk factor reduction interventions.
  • Review of age/gender appropriate screening tests.

Biometric tests required include:

  • Blood draw for cholesterol (TC), HDL cholesterol, LDL cholesterol, glucose, A1C and triglycerides.
  • Blood pressure.
  • Height, weight, body mass index (BMI).

Employees will need to download the Healthy Boiler Center for Healthy Living Provider Screening form and take it to their annual physical. The form is available on the Healthy Boiler portal, www.healthyboiler.com, under the “Earn” tab. The primary care provider will need to sign it – including the values – before the form is uploaded to the Healthy Boiler portal as documentation of completing this step.

The information from the physical and the results from the biometric screening will be used by the Center for Healthy Living to provide employees with information to help understand current health and potential risks, and may also be used to offer services through the wellness program such as but not limited to health coaching, medication therapy or weight management. Employees also are encouraged to share results or concerns with their own primary care provider. The only individual(s) who will receive personally identifiable health information are One to One Health physicians, nurse practitioners, and pharmacist, dietitian or health coaches in order to provide employees with services under the wellness program.
