Customer Service Form

This form is intended for the submittal of non-critical facilities requests and inquires. Our staff will follow up with you as soon as possible during normal business hours Monday through Friday. 

To submit a maintenance request, use the Fix It service request portal.

If your request is related to confidential carts, please complete the Confidential Cart and Recycling Container Request Form. 

Please use one of the following methods to communicate a critical or time-sensitive Physical Facilities issue.

• From 7:30 a.m.– 4 p.m. contact the Work Request Center at (765) 494-9999.
• From 4 p.m.– 7:30 a.m. contact the Purdue Public Safety Dispatch Center through their non-emergency phone number at (765) 494-8221.
