Please see below for instructions on how to view PDFs on the web, followed by a list of forms used by the Office of the Bursar. Copies of these PDF forms should be copied, completed and forwarded to the Office of the Bursar. If you are unable to print a form, please contact that office for a copy.

- Student Release Form (PDF): A form for students to fill out and submit to the Office of the Bursar, requesting a release of information regarding tuition, fees or housing.
- Outside Award Donor Form (PDF): A form for students to fill out and submit to donors.
- W-9S (PDF): An Office of the Bursar request form asking for the Social Security number and certification of a student or borrower.
- Audit Fee Remission Form (Form 15A) (PDF): A fee remission application form for staff, for use when auditing a class.
- Purdue University Global Remission Form (Form 15G) (PDF): A fee remission application form for Purdue University Global staff, children of staff members or spouses of staff members who are attending school at the West Lafayette campus.
- New Albany Polytechnic Remission Form for Nonresidents (PDF): A reciprocal tuition application form for Kentucky residents attending classes at the New Albany Polytechnic location.
- Richmond Polytechnic Remission Form for Nonresidents (PDF): A reciprocal tuition application form for Ohio residents attending classes at the Richmond Polytechnic location.
- Cash Receipt Voucher (PDF): A cash receipt voucher form, used for department deposits.
