Reporting Concerns

The Dean of Students wants every Boilermaker to be successful. To ensure the success of the entire community, we sometimes need to let someone know when we have concerns. This section provides helpful links to experts in handling an array of student concerns.

• Student Of Concern Report — Faculty, staff, students or others who are concerned about a student's behavior are encouraged to report the concern to the Office of the Dean of Students using this form. Please contact the Purdue University Police Department for immediate concerns regarding safety (765-494-8221) or Counseling & Psychological Services for mental health crises (765-494-6995). 
• General Incident Report — Anyone within or outside the Purdue community may use this form to report an incident to the Office of Student Rights and Responsibilities. 
• Academic Dishonesty Report — Instructors may use this form to report incidents of academic dishonesty to the Office of Student Rights and Responsibilities. Any student reporting academic dishonesty should use the General Incident Report form above. 
• Purdue University Office for Civil Rights Incident Report — Anyone within or outside the Purdue community may use this form to report to the Purdue University Office for Civil Rights an incident that may violate the Anti-Harassment Policy and/or the Equal Opportunity, Equal Access and Affirmative Action Policy or the Title IX Harassment Policy.
