New ID cards for medical, dental and health funding accounts to arrive for 2022

Benefits-eligible employees will receive new identification cards for 2022 from the following:

  • Anthem (medical insurance)
  • Delta Dental (dental insurance)
  • HSA Bank (HSA ,FSA, HRA

Employees are encouraged to verify that their mailing address and other personal information is up-to-date in SuccessFactors to ensure their new ID cards reach them in a timely fashion.

The 2022 Anthem ID cards will have a new group number and member number, which means that employees will have to show their Anthem ID card to their medical providers and pharmacy at their first visits in 2022.

Delta Dental will issue two ID cards (regardless of how many dependents re enrolled) per employee for dental insurance in 2022. Note: Both ID cards will be in the employee’s name, but all those covered on the dental plan are able to use the cards, which need to be presented at dentist appointments throughout the year. To order additional cards, employees may call Delta Dental's customer service at 800-524-0149 or can print additional cards through the Member Portal or Mobile App via their phones.

Many HSA Bank cards are expiring at the end of the year for those who have had a card since 2018. Renewal cards are automatically triggered 45 days prior to expiration, so new cards will start arriving in home mailboxes at the end of November, and all should be received by mid-December. For those new to HSA Bank, welcome letters and ID cards should be received by the end of December. HSA Bank Cards are sent in plain envelopes for security reasons.
