Telehealth
Telehealth, a new integrated program led by the CHL, provides convenient and timely access to acute care and healthcare consultation. You now have a dedicated care team that is accessible to you 24/7 via text message, video chat or phone call. Offering unlimited access to the care, you can receive treatment for acute needs, prescriptions, medical advice, appointments, referrals and timely follow-up, all at no cost to you.
If you have a healthcare-related question, need or concern, you can text the CHL Telehealth team at 765-705 0584 and receive care or download the TextCare app to communicate with the telehealth team.
All benefits-eligible Purdue employees and dependents, age 2 and above, covered on a Purdue medical plan are eligible to use the CHL Telehealth Program.
Mary Evangelista, FNP-C – Mary is a 2008 Purdue grad and has been caring for patients as a nurse and nurse practitioner since 2011. She lives in Crown Point and has experience in both emergency medicine and family practice. She enjoys spending time with her husband, daughters and their two dogs (a black lab/pit rescue and a cavalier spaniel).
Amanda Vagelatos, FNP-C – Amanda loves the CHL Telehealth program because of the collaborative approach to delivering quality care and connecting with patients wherever they are in their day. When she’s not working, Amanda loves spending time with her husband and their three children. They spend a lot of time at sport practices and games. She and her family also love to travel, especially to the beach.
Think of the CHL Telehealth team as your front door to healthcare. If you have a healthcare related question, need or issue, you will be able to simply text them and receive care. The team can conduct video visits, write prescriptions, offer advice, scheduling support, benefits navigation and referrals as necessary.
Simply text the CHL Telehealth team at 765-705-0584. The care team will ask each new patient to complete a Registration Form which initiates HIPAA compliant consent documentation for communication via text, phone and video calls as well as providing important health history. You may also choose to download our TextCare app to communicate with the telehealth team.
Mary and Amanda are available 8 a.m. to 5 p.m. EST with back-up support provided 24/7 from the One to One Health Virtual Care Team. If you contact the team after hours, you will likely get a different licensed medical provider, but they will coordinate with Mary and Amanda to ensure continuity of care and appropriate follow-up.
There are no co-pays, visit charges, bills or insurance charges of any kind for 2024. If the CHL Telehealth team refers you to in-person care, you will be billed as normal for any in-person care received.
