Earn a Cash Wellness Incentive

Taking care of your health is important and can lead to better quality of life.

Incentive funds can be earned by completing your annual physical. Additional funds can be earned by also completing your biometric screening. The annual physical (step 1) is required to receive incentive funds, and the biometric screening (step 2) is optional.

Employees who complete their annual physical (and biometric screening if they choose) and submit their physical form to Vital Incite – third-party data analytic vendor – will receive a cash incentive as outlined below in the chart.

Amount awarded is based on coverage level elected within a Purdue Global medical plan. Employee&Spouse and Family levels – both employee and spouse may earn independent of the other. Employee&children – only the employee needs to complete the activity to earn amounts. Employees who opt out of the medical plan may earn employee-only amounts.

Incentive Payment

Payment is made by the end of the month following the month in which the program requirements are completed. The funds will be deposited via payroll.

A report in aggregate will be provided to Human Resources indicating how much each person should receive based on completed items. Human Resources will not receive any personal health information related to your incentive completion.

Ex) A person who fulfills the wellness program requirements by the end of April, including submitting their information to Vital Incites, will receive incentive dollars in May.
