Questionnaires Overview

The IACUC Protocol Forms, also known as “Questionnaires”, are dynamic lists of questions which are replacements for some of the traditional IACUC hardcopy forms.  Each answer entered on a questionnaire will automatically navigate the Investigator to the next required question.  By this method, only those questions applicable to the specific submission will be presented to the Investigator and all unnecessary questions will be excluded.  This will reduce Investigator burden and simplify the completion of protocol submissions.

Questionnaires are required for completion when available and will automatically display when appropriate.  There is no need for an Investigator to spend any time searching for or determining the appropriate questionnaire to include with their submission.  At this time, Questionnaires are only being utilized for Annual Continuation and Triennial Renewal submissions.

Questionnaire Completion

Answers to Questionnaire questions may be yes/no, value list, searches, or short answer.

NOTE:  You may Save a Questionnaire at any point, exit or navigate to a different section, then retun later to complete.

1.  Click either "Annual Continuation" or "Triennial Renewal Suppliment" under the "Questionnaires" heading, in the left navigation

2.  Answer each question that is presented

3.  Click “Save & Proceed” when all questions on a screen have been answered

4.  The system will inform you when the Questionnaire is complete
