Dean Certifications and Student Conduct Checks

The Office of Student Rights and Responsibilities maintains the non-academic record of students referred to the university who are in some way involved in a potential violation of University Regulations. Dean Certifications are forms or requests that are completed by the Office of Student Rights and Responsibilities certifying whether a student has or had a disciplinary record during their time at Purdue University on behalf of the Dean of Students. These forms may be completed for the following reasons:

  1. Transfer applications
  2. Graduate school applications
  3. Medical school applications
  4. Law school applications
  5. Any other applications that may require this information (including potential employers and federal investigative agencies)

Disciplinary records are educational records protected by the Family Educational Rights and Privacy Act (FERPA). The Office of Student Rights and Responsibilities does not share information regarding disciplinary records with anyone outside of Purdue University without a student’s written consent, except in certain limited circumstances. All forms must be completed with necessary student information prior to submitting for completion by the Office of Student Rights and Responsibilities. Failure to complete all required fields may result in a delay of processing.

The Office of Student Rights and Responsibilities will send completed forms to the appropriate third-party whether through mail, e-mail, or fax. If the form does not indicate where the form should be sent to, the student or third-party must supply this information to the Office of Student Rights and Responsibilities at the time of the form submission.

Please allow up to five (5) business days for the Office of Student Rights and Responsibilities to complete all requests.
