Forms and Documents

The Office of the Registrar provides access to various forms for your convenience.  If individual forms require specific instructions, you will be directed to an information page before your download begins.

 

Letters of Recommendation Release Form

Used to comply with FERPA regulations when a student requests a letter of recommendation that includes personally identifiable information from their education record.

This form must be completed in full and retained by the recommender or department for one year.

 

Form 340: Personal Information Change

Used to request changes to personal information on a student’s official record. When filling out the request form please note our office can only accept wet or Adobe signatures.

Request to Restrict/Unrestrict Directory Information

Allows students to restrict or remove restriction on their directory information. In-person visits are encouraged to understand the implications.

Select and complete the appropriate form. Refer to FERPA Basics for Students webpage for instructions.

 

Form 23: Schedule Revision Request

Used to request schedule changes when the Scheduling Assistant does not allow modifications.

Obtain from your Academic Advisor or Graduate Registration Contact. Submission information found on form.

 

Form 60: Incomplete Grade Agreement

Used to outlines the deadline, remaining requirements, and grading expectations for a student to earn a final grade.

Completed and submitted by the faculty of record. Retained internally by the department; not collected by the Office of the Registrar.

 

Form 65: Incomplete Extension Request

Used to request an extension for completion of an incomplete course.

Should be completed and submitted by the faculty of record on behalf of the student. Submission information found on form.

 

Directed Credit Form

Requests non-registered credit to be articulated onto the student record.

Submission information found on form.

 

Excess Credit Form

Used to request a change in course level for applying a course toward a graduate plan of study.

Submission information found on form. To be submitted after the undergraduate degree has been awarded.

 

Include/Exclude Form

Used when a non-repeatable course is retaken and prior attempts are not properly excluded from the GPA. If different courses are involved, include a memo signed by the department head with a plan to request a permanent course number.
